On : 12.2.1.3.0 version, Default
When attempting to run a form , the following error occurs immediately after entering user credentils and hitting Ok.

Forms Session ID is WLS_FORMS.formapp.xx
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Access the form
2. Enter the credentials and hit Ok.
3. See the Forms error thrown.
Note that sometimes the form appears for about 1 millisecond before error is thrown.
