Unable to Start New Child JVM After Crashing Existing Child JVM (Doc ID 2918060.1)

Symptoms

 After a child dejvm process terminates abnormally, subsequent child dejvm processes fail to start, even though the parent JVM Controller process is still alive.
The JVM Controller log (debug level) records the following error:

"Error: JVM Controller with same name already running."
