JDBC Datasource Monitoring Tab Does Not Show the Target WebLogic Server in Console (Doc ID 2919931.1)

Symptoms

JDBC datasource is not showing up its target weblogic server on the console page.
Database ping to host/IP works as expected.

The issue happens only when any changes to the datasource configuration are done and the managed server is restarted.
If the managed server is not restarted, it shows up as a target under the datasource monitoring tab.

Below error is seen in the logs:
