ORA-20100 or ORA-20999 Errors while Running Reports using SRW Package (Doc ID 292428.1)

Symptoms

Using Event-driven API to run a report from the database level displays the
following errors:

ORA-20999:
ORA-06512: at "SRW", line 284
ORA-06512: at "SRW", line 815
ORA-06512: at "PROCEDURE", line 131
ORA-06512: at "RUN_REPORT", line 5
ORA-04088: error during execution of trigger 'RUN_REPORT'

OR

ERROR at line 1:
ORA-20100: Error occurred while parsing: Attribute value should start with quote.
ORA-06512: at "SYS.XMLPARSER", line 22
ORA-06512: at "SYS.XMLPARSER", line 86
ORA-06512: at "PORTAL.SRW", line 333
ORA-06512: at "PORTAL.SRW", line 813
ORA-06512: at "PORTAL.PROCEDURE", line 17
ORA-06512: at line 1
