Unable to Access WebLogic and EM Consoles Through Load Balancer Failing with "connection timeout" (Doc ID 2936161.1)

Symptoms

Installed a fmw_12.2.1.4.0_infrastructure on an application server behind a Load Balancer.
Console and em can be accessed directly from the server with no problem. However going through Load Balancer from corporate network, the console login page is seen.
After entering the username and password credential, it doesn't come back, eventually it ends up a "connection timeout" after 40 seconds.
