Portal Server Connection Creation Fails with "URL is not reachable. Verify that the url points to a valid admin server host and port"

Symptoms
Receive error when attempting to create a Portal Server Connection from a source environment to a target environment. In the user interface, the following is displayed:
URL is not reachable. Verify that the url points to a valid admin server host and port
The WebCenter Portal diagnostic log shows:

java.io.IOException: Failed to initialize JNDI context, tried 2 time or times totally, the interval of each time is 0ms.
Could not establish a connectionManager with <ID>:<TARGET_HOSTNAME>:[<ADMIN_PORT>,-1,-1,-1,-1,-1,-1]:inetAddress=<TARGET_HOSTNAME>/<TARGET_IPADDRESS>:wcp_domain:AdminServer, t3://<SOURCE_HOSTNAME>:<ADMIN_PORT> java.rmi.ConnectException: Destination <TARGET_IPADDRESS>, <ADMIN_PORT> unreachable.; nested exception is:
java.net.ConnectException: Connection refused; No available router to destination.; nested exception is:
java.rmi.ConnectException: No available router to destination.; nested exception is:
