OIDDAS Console Failing with "Error encountered while connecting to Directory Server." after OAM Upgrade from 11g to 12cR4 (Doc ID 2957986.1)

After upgrading from OAM 11g to OAM 12cR4, OIDDAS console is no longer connecting to the configured OID LDAP server. 

Upon logging into OIDDAS console, the following error message is seen: "Error encountered while connecting to Directory Server."
