After Schema Upgrade the SCHEMA in SCHEMA_VERSION_REGISTRY Do Not Show As Upgraded (Doc ID 2990419.1)

Symptoms

After Schema Upgrade the SCHEMA in SCHEMA_VERSION_REGISTRY Do Not Show As Upgraded

Ran Upgrade Assistant to upgrade schema from 12.2.1.3 to 12.2.1.4. However, after running Upgrade Assistant, when we go to verify the schema per the following documentation:
- https://docs.oracle.com/en/middleware/fusion-middleware/12.2.1.4/wccup/upgrading-webcenter-sites-previous-12c-release.html#GUID-27DF9569-BB7D-4021-AAE0-88E9010D9B12

It appears to not be upgraded or the correct version:
