Forms Application Fail with "FRM-93240: Multiple Forms applications cannot share an HTTP Session" after Applying OSS Bundle Patch (Doc ID 3009884.1)

Accessing the Forms application via Load balancer gives below error:

FRM-93240: Multiple Forms applications cannot share an HTTP Session

This is the request flow:

User --> LB --> OHS --->WLS-Forms

The same application works fine when accessed directly using WebLogic host/port or when accessed via OHS (skipping LB).

Below errors are noticed in OHS error log when the application is accessed via Load balancer.

All the latest PSU patches are applied on Forms/Weblogic/OHS/Infrastructure.
