SOA 12c Upgrade Process Does Not Upgrade All Instances (Doc ID 3027650.1)

Symptoms

The issue was noticed during the process of in-place upgrade from SOA version 11.1.1.9 to 12.2.1.4.

As a result of running upgrade assistant to upgrade the existing SOA composite instances (opened and closed), some errors have occurred similar to that one reported in Doc ID 2826368.1. The solution from this document was applied and the upgrade job has been restarted. When the job has completed it was noticed that not all the instances were upgraded and even some of them were opened instances. They were identified in COMPOSITE_INSTANCE database table as having flow_id null, using, for example, the following SQL query that shows also the state of the instance:
