Symptoms

On : 12.2.1.3.0 version, SQL*ReportWriter

You are running Oracle Reports server 12c and your configuration uses a cgicmd.dat file with keys for database connection information. Most of the time it works fine, but during a batch load of report requests, the occasional report fails. 

The rwservlet_diagnostic.log reports this failure as follows:
