NullPointerException in WebLogic Code After Message "RequestContext had not been properly released on earlier request" (Doc ID 3068616.1)

Symptoms

An ADF application was migrated from 11g into 12.2.1.4.0.
Logs were filled with different types of errors, and the one that affects their users is a NullPointerException in WebLogic code.

Every time this error occurs, the logs include a warning about the session:

Request Context had not been properly released on earlier request
