OracleJSP: java.io.FileNotFoundException when clicking on initiate process tab of the bpel console (Doc ID 353432.1)

Symptoms

Within the Dashboard tab, clicking on TaskManager in the Deployed BPEL
Processes column, takes you to initiate tab of the Process, and shows the
following error -
 OracleJSP: java.io.FileNotFoundException:
 Set the init-param debug_mode to "true" to see the complete exception message.
 
This also occurs with the Descriptor tab
