Symptoms

Background
- When starting SqlPlus and attempts to logon, SqlPlus crashes with the error:

Event identifier: None
Description: The application "<ORACLE_HOME>\ bin \ sqlplus.exe" has
caused a program error. Date and time of the error: <time> ...
Exception: c0000005 to address <address> (<nosymbols>).
