SSO Logout Does Not Log You out From Any Mod_OSSO Midtier App

This problem can occur on any platform.
SSO Logout does not log you out from any mod_osso middle tier application.
The error_log file shows the following:
[Wed Feb 08 17:10:38 2006] [error] [client <IP_ADDRESS>] [ecid: <ECID>] File does not exist: <ORACLE_HOME>/apache/apache/htdocs/osso_logout_success
mod_osso registration took place recently. And, since then, no logout is working correctly from that middle tier.
