Unable to Login to /pls/orasso and SSO Logout Fails

Oracle AS Single Sign-On - Version: 9.0.4 to 10.1.2.0.2
Unable to display and/or login to /pls/orasso
Are able to login successfully to /oiddas, but unable to logout from any application e.g. /oiddas or /pls/portal.
error_log file shows the following:
Proxy log On failed.Please verify that you have specified correct connectivity information i.e. username, password & connect-string in the DAD
Anonymous binds disabled in Oracle Internet Directory (OID).
