Custom Application Protected by Java SSO does not Prompt for User Credentials when Accessed

Symptoms
Java SSO is configured correctly in custom OC4J container.
New realm user 'user' was created for this container under 'myrole' security role.
Test login for Java SSO is successful using the 'user' user.
A custom application 'myapp' deployed to the same container as 'javasso' application .
The 'myapp' has been marked as Java SSO participating application.
All of these configurations have been done through Application Server Control.
When the 'myapp' is accessed, the end user is granted automatic access to application
without prompting for user credentials.
