Oracle Access Manager Operation Error "Login Correspond to more than One User Profile" During Login (Doc ID 416770.1)

When attempting to login into an application that is protected by OAM WebGate, the following error occurs:

Oracle Access Manager Operation Error
The credentials (userid=orcladmin password=(omitted) Resource=/xxxx/redirect.html
RequesterIP=1xx.xx.xx.xx HostTarget=http://host.domain:port Operation=POST) used in the login correspond to more than one user profile in the Identity System. The correspondence must be unique.

Steps To Reproduce:
The issue can be reproduced at will with the following steps:
1: Enter the protected application URL in the broswer
2. User is prompted for credentials, enter the account and password in the input fields.
3. Press login button and the error occurs.
