Can't Run Form From Desinger After Applying Forms Patch 5017052 (Doc ID 423566.1)

Symptoms

After Forms Patch 5017052 is applied to the home directory of Internet
Developer Suite.
Internet Explorer browser will not run a form when the form is generated from
Designer.

o The loss of functionality - unable to run a form directly from Designer
o The sequence of events- 1. Generate form from designer and the web page is blank
o Where it happened - This has been reproduced internally
o The significance of loss - takes more steps to generate a form from designer
o What IS working: The form will generate if it is generated and then run from forms builder.
o What is the expected or acceptable result? : Run from the Designer application.
