Apex Failed To Load Definition Page and Receives HTTP/1.1 304 Status Code in Access Log File After Install / Upgrade (Doc ID 438792.1)

Symptoms

After upgrading APEX fails to load the definition page.  

When checking the access_log.<timestamp>, it shows "HTTP/1.1" 304 status errors similar to:

[05/Jul/2007:22:32:06 -0700] "GET /i/htmldb/icons/create_small.gif HTTP/1.1" 304 -
[05/Jul/2007:22:32:06 -0700] "GET /i/htmldb/icons/pe_report.gif HTTP/1.1" 304 -
[05/Jul/2007:22:32:06 -0700] "GET /i/htmldb/icons/updown_arrows.png HTTP/1.1" 304 -
[05/Jul/2007:22:32:06 -0700] "GET /i/htmldb/icons/cursor.gif HTTP/1.1" 304 -
[05/Jul/2007:22:32:06 -0700] "GET /i/htmldb/icons/pt_validation.png HTTP/1.1" 304 -
[05/Jul/2007:22:32:06 -0700] "GET /i/htmldb/icons/pt_theme.png HTTP/1.1" 304 -
[05/Jul/2007:22:32:06 -0700] "GET /i/htmldb/icons/pt_security.png HTTP/1.1" 304 -
[05/Jul/2007:22:32:06 -0700] "GET /i/htmldb/icons/pt_nav.png HTTP/1.1" 304 -

Changes

APEX has been installed / upgraded on the current system.
