Info for 'Last Action' missing from My Workflows page

Symptoms
When customer goes into My Stellent -> My Workflows, the workflows that are currently assigned to the user appear, but the info in the "Last Action" column only appears for some of the workflow steps
Why do actions show for only some steps of the workflow?
