"Cannot insert duplicate key in object 'Revisions'" Error When Attempting to Check In a Content Item (Doc ID 449430.1)

When attempting to check in a content item into the Content Server, the check-in fails with the following error:

Content Server Request Failed
Unable to execute service CHECKIN_NEW and function Irevision.
(System Error: Unable to execute query 'Irevision(INSERT INTO Revisions (dID, dDocName, dDocType, dDocTitle, dDocAuthor,
dRevClassID, dRevisionID, dRevLabel, dIsCheckedOut, dDocAccount, dSecurityGroup,
dCreateDate, dInDate, dOutDate, dStatus, dReleaseState,
dPublishType, dPublishState, dWorkflowState)
