Reverse-Proxied OAM Form Login with Passthrough Enabled Fails with HTTP-502 Proxy Error

Reverse-proxy WebGate 10.1.4.0.1 with form-based login. Anonymous authentication scheme for login/logout pages. Backend webserver is IIS 6.0, no SSL configured.
When form-based login is implemented with passthrough:yes with a WebGate reverse-proxy, login fails with error from the Apache WebGate: HTTP-502 Proxy Error.
With passthrough:no proxy form login is successful.
Without proxy configuration - reverse-proxy serves all pages locally - form login is successful with both passthrough:yes and passthough:no.
