Symptoms

There are two Oracle Application Servers 10.1.3.3 installed in a cluster environment using a hardware load balancer.
Running a test to see how Oracle would handle failover when one of the OC4J instances on one of the machines is brought down,
it was expected that it would failover to the live instance on the other machine that is part of the cluster.

But instead , it did not, receiving the following error in the OC4J log file:
