This problem can occur on any platform.
The actionable email is received in your Lotus Notes Client so you proceed to click one of the worklist options (YES or NOT); sending an email with your response to the configured actionableEmailAccountName.
The email is received and consumed out of the account, the problem is that BPEL Server is not able to process the response contained in the email therefore the Human Task continues to wait for a response despite the fact that the email reply has been already been sent.
If you turn on the debug mode of the loggers of the BPEL Admin Console (Not the loggers of the BPEL Console), you can see this error.
