User not redirected to a locked out page/message after upgrading to 10.1.4.x (Doc ID 743393.1)

This problem can occur on any platform.

Symptoms

In OAM 6.x, when a user is locked out they is redirected to 
 
http://<host>:<port>/access/oblix/apps/webgate/bin/webgate.dll?status%3D500%20errmsg%3DErrUserLockedOut... 
 
which displays the message from webgate.msg. 
 
After migration to 10.1.4 a locked out user is no longer redirected to the above location. Instead, the locked out user continues to receive a login prompt.  However the locked out user will not be able to login (even with good password) until the lockout duration has expired. 

The HTMLpage500 tag in webgate.xml is not invoked for locked out scenario as it was prior to upgrade.
