OWA_SEC Functions Do Not Return Anything or Fail with ORA-01403 Error

This problem can occur on any platform.
Symptoms
When mod_plsql Basic authentication is used, calling the following OWA_SEC functions:
- owa_sec.get_client_hostname
- owa_sec.get_user_id
- owa_sec.get_password
does not return anything.
Furthermore, owa_sec.get_client_ip fails with an ORA-01403 error.
Errors similar to the following are displayed in the HTTP Server error_log file:
