FRM-92102 Error When Accessing Forms Application Through Peoplesoft Portal (or separately in same browser) (Doc ID 743977.1)

Symptoms

When attempting to connect to Forms from a PeopleSoft Portal, or Forms and Peoplesoft separately but in the same browser, the following error occurs:

ERROR Displays FRM-92102 error and 'attempts to reconnect 5 times' message.

The same issue occurs if using "separately started" IE8 browsers.
