HTTP-404 Occurs While Running Forms With A Newly Created OC4J Instance For Forms Services (Doc ID 753108.1)

Symptoms

On 10.1.2.0.2 in Production:
Created a new custom OC4J for Forms and deployed formsapp.ear file as per "Oracle Forms Services
10g Advanced Configurations" "An Oracle Technical brief ".
Not able to use new vitual mapping for a directory based on the new OC4J mount point.
Accessing the form using new OC4J fails with "HTTP-404 Page Not Found" error.
