Symptoms
Upon initialization of a .NET custom action modifier, the following error is being received:

A failure occurred while attempting to initialize one or more tools!
InitInstance failed in CustomActionModifier
Reported error: Class: CustomActionModifier
Assembly: c:\Program Files\Stellent\IBPM\CustomActionModifier.dll
Did not evaluate to a System.Windows.Forms.Form type.
