CSV Export of SQL Report Omits Some Columns in Exported File

A SQL Report is created and executed successfully. When trying to export it to a spreadsheet, some of the columns are not exported or not seen in the spreadsheet.  All the columns of the table have the property "Include In Export" as 'Yes' on in Reports Attributes page:

Home>Application Builder>Application>Page>Report Attributes>Report Export

Each of the individual columns have the "Include In Export" property set to  'Yes':
