Patches Not Working Even After Applying Them Properly (Doc ID 811975.1)

Symptoms

OSB/ALSB patches are applied through bsu/smartupdate (till OSB 10.3.1) or through opatch (from OSB 11.1.1.3 onwards) are not taken into effect. Applied patch(es) are supposed to fix the issue for which the patch was delivered.
