PDFConverter Errors On Conversion: csPdfOIXError2, and FileNotFoundException for DocConverterResults.hda (Doc ID 884383.1)

Symptoms

When PDFConverter is installed on the Inbound Refinery, an error occurs on startup of refinery and on conversion of documents.

The first error below is from the refinery startup, and the second occurs when a document (.doc) is converted to PDF. The content item is not successfully converted.
