Error: Page Metadata Could Not Be Obtained From The Portal Database (Doc ID 887957.1)

Symptoms

Attempting to access Portal through a reverse Proxy and the following error occurs in the browser:  

Error: Page Metadata could not be obtained from the Portal database.    


The OC4J_Portal application.log contains the following errors: 

portal: id=<ECID> ContentFetcher Unexpected Exception Request
Failed:oracle.webdb.dispatcher.DispatcherException: I/O Error dispatching request
name=content-fetcher5 label=pageMeta
url=http://<HOSTNAME>:<PORT>/portal/pls/portal/!PORTAL.wwpob_page.show/TOPLEVELSITE
time=60002ms timeout=60000ms process=Dispatching    

  

Ping and Telnet to the hostname/virtual hostname used to access Portal is successful.
Portal configuration is front ended by a reverse proxy (Apache 2.2).

Note:
This occurred in a Portal 10.1.x environment, but similar errors could also occur in a Portal 11.1.x environment.
