Policy Manger Is Not Able To Add New Policy In Policy Domain

Issue with Oracle Access Manager's (OAM) Policy Manager (PM).
When adding a policy to a specific Policy Domain (PD). No error is thrown, it returns as successful, but the policy is not added.
The PM log file contains WARNING level logs... A problem in the ldap_modify call has been dectected. A few lines above the WARNING messages are ldap calls indicating the response from the directory was LDAP_PROTOCOL_ERROR.
