Symptoms

The following documentation has been followed in order to setup a Fault Policy:

FUSION MIDDLEWARE DEVELOPER'S GUIDE FOR ORACLE SOA SUITE 11g Release 1 (11.1.1)                            

Part Number E10224-01

Under Section 12.4, "Using the Fault Management Framework" in Example 12-3 and Example 12-4.

After following this you find that the fault policy does not work.
