FMW 11g High Availability Topology - "Error flushing write connection to OPMN" in Admin Server Diagnostic Log (Doc ID 967270.1)

Symptoms

Two or more Fusion Middleware 11g Instances reside on the same machine as part of a high availability architecture. This might be an Identity Management or 'Portal/Forms/Reports/Discoverer' High Availability topology whereby one machine hosts an Administration Instance as well as one of the Runtime Component Instances.  

The AdminServer-diagnostic.log shows the following error repeated over and over again.
[ERROR] [] [oracle.as.management.mbeans.opmn] [tid: DmsThread-1] [userId: fmwweblogic] [ecid: 0000IIW7dSE6ATd5xfP5iW1AuO1l000004,0] [APP: DMS Application#11.1.1.1.0] OPMN.queryProcessStatus failed with the following exception[[
oracle.as.management.opmn.optic.OpticBadConnectException: Error flushing write connection to OPMN
If the Weblogic Admin Server has been started from the console / command prompt using - startWebLogic.sh / startWebLogic.cmd then the errors will be seen in the command prompt / console window.
