Symptoms
When using forkable portlets, the image theme path is not correct as specified in the skin.properties. In the default skin.properties file, there are settings like:
theme.plain.search.path: plain/images, images
theme.alert.search.path: alert/images, images
Therefore, if a portlet is using the alert theme then the toggle-button images should be primarily taken from the "/framework/skins/default/alert/images" directory.
This works if the portlet is not forkable.
But if the portlet is forkable then the default images are taken from the wrong directory which is:
/framework/skins/default/images
