Site Studio Contributor Mode Displays Standard Checkin Form and Not Profile Trigger Form (Doc ID 986672.1)

Symptoms

Using Site Studio 9.0.0.378 on Content Server 10.1.3.5.1, the profile trigger option does not work in Site Studio contributor. When users create a hyperlink in Site Studio contributor mode and choose "Link to a file" > "New Contributor data file"  the checkin form that displays is the standard checkin form and not the one that should display as per the profile trigger.
