Box 8 on the VAERS Form is Not Getting Populated with "Other" and the Text Entered in Other Text (Doc ID 1052085.1)

Symptoms

The system is not populating the Box 8 on the VAERS form with "Other" and the text entered in other text when Other check box is checked in the seriousness criteria section of the event tab. This is causing the form to appear as it would in a non-serious report.

Steps to Repeat:
1. Open a vaccine case
2. Go to events tab and check Other check box in the seriousness criteria section and enter "AERS report" in the other text field
3. Schedule a VAERS report and save the case
4. View the scheduled draft VAERS report
5. Observe that it checked "None of the above" check box in Box 8 on the VAERS form but did not populate with the word "Other" and the other text right below to the "None of the above" check box. This is making the form to appear as it would in a non-serious report.
