FRM-92101: There was a failure in the Forms Server during startup trying to run a Report or Upload a Document in AERS 4.6.1.x

Recently installed AERS environment running AS10GR2PS2.

AERS is crashing with the following errors when accessing the application to upload documents to 
a case or alternatively when attempting to access the form to run any report e.g.
FRM-92101: There was a failure in the Forms Server during startup. This could happen due to invalid configuration.

Please look into the web-server.log file for details


After performing a forms trace, the FRD.Log is showing 27-JAN-2010 20:37:28 TESTAERS1 -(Null)
Error Message: FRM-40401: No changes to save.

/Forms encountered unexpected signal -1073741819

The frmweb_dump_5476 file is showing:

The error does not appear to reproduce if running a simple form like the test.fmx supplied as part of the
Default Oracle AS install e.g.
