Unable To Generate DCI Forms in Oracle Clinical - Job Hangs in Reports Server / Forms Session Appears To Hang Too. (Doc ID 1072412.1)

The Oracle Clinical forms session appears to hang when trying to generate a DCI Form in Oracle Clinical.
The same DCI form can be generated successfully after killing the forms session but the problem then moves to another DCI Form.

The Reports Queue Manager shows the job as running / has a status of '(<reports server name>) Opening '
but never seems to complete. Since this job does not complete then control is not returned to the forms
session / results in the hang.
