'ORA-20000: 822100-The User <ops$username> Is Maintained In Oracle Clinical'

Symptoms
You created two Dictionaries in TMS 4.6. In Dictionary Link tab, when you try to do a Cross Dictionary Link from one dictionary to the other, the LOV which should have the dictionaries listed, has no values FRM-41830: List of Values contains no entries
Navigation: Definition > Define Dictionaries > Dictionary Link Tab
You also determined that that the dictionaries are only available to users that were created in TMS. Users that were created using the OCL_ADD_USER.SQL receive the error:
FRM-41830: List of Values contains no entries
Upon checking the Define User screen you cannot query up (Press F8 ) the user <ops$username>.
When you click on 'Get O* Clinical Data' from Define User screen in TMS, you receive the following error:
ORA-0001: unique constraint (OPA.OPA_ACCOUNTS_UK1) violated
ORA-06512: at line 1
ORA-06512: at "OPA.OPA_USER_MT_SCHEMA",line209
FRM-40735: STD_OPTION1 trigger raised unhandled exception ORA-00001.
Also when you try to create a new user in TMS you receive the following error message:
'ORA-20000: 822100-The user <ops$username> Is Maintained in Oracle Clinical'
OCCURRENCE PATTERNS: consistent since creating the users via OCL_ADD_USER.SQL
