Symptoms

Very often you have a valid working SQLPLUS session and you are able to send commands to the database server, for example:

SQL> select username,machine,count(*) from v$session group by username,machine;
USERNAME MACHINE COUNT(*)
------------------------------ ------------------------------ ----------
OSM01TORONTO 33
SYS CA-ORACLE\bniu-lap 1
OSM631_53_TT CA-ORACLE\bniu-lap 2
OSM631_B53_W OSM01TORONTO 1
PROVISIONING_POL_TM labdb2.ch.oracle.com 2


Then all of a sudden it is not working anymore

SQL> /
select username,machine,count(*) from v$session group by username,machine
*
ERROR at line 1:
ORA-12571: TNS:packet writer failure
