Mysterious Login Attempts By Onsite Application Server Or User's PC

In the DBA report of failed login attempts to the application, a user account showed up as having tried to login, but the IP address points to the Onsite server. The user was away from the computer at the time and didn't try to login at all.

Is there something on the Onsite application server that will prompt an auto login?
