Telephone Number Stuck in 'Pending' Status After Application Improperly Unassigned it From PSR Order

Symptoms
Telephone number stuck in 'Pending' status after application improperly unassigns them from the PSR order. The problem occurs when attempting to remove a telephone number from the order via the 'Inventory and Assign' tab of the 'Assign Telephone Numbers' window (un-assign ) that was already sent to the 911 provider. The application displays two error messages (see below), but removes the telephone number anyway and leaves it in a 'Pending' status.
ERROR 1:
"Error in procedure.
SP_DELETE_ASSIGNMENT(180:E911 has been extracted from this number. You can not delete this number.)."
"Error in procedure.

ERROR 2:
"Error unassigning telephone number XXX-XXX-XXXX
Please contact support."
