Cannot Run Any Report; Error- Frm - 41214 Unable To Run Report (Doc ID 1264123.1)

Goal

Recently starting getting the following error submitting any REPORT per AERS or TMS:

FRM - 41214 Unable to run report



The report trace showed error.
eg:
the rwserver.trc showed error:   

'Oracle Portal is not connected!'



 but portal password was valid and working.

Notes:

  1. The Portal password is still valid and non-expired and it was verified that could successfully log into PORTAL via SQL*PLUS  (using ldapsearch password).
  2. The corresponding <reports>.conf file has the correct password too, and verified per:
    $ORACLE_HOME\reports\conf\<report_server_name>.conf
