Symptoms

If pin_mass_refund is run twice, it creates a new /item/refund object each time.
 
Steps to reproduce

1. Create an account.
2. Create an adjustment for $10.
3. Run pin_mass_refund, /item/refund is created and $10 is transferred from /item/adjustment.
4. Run pin_mass_refund again, a NEW /item/refund is created and $10 is transferred in from existing /item/refund.
