Policy Services UI login fails due to DSEE instance schema corruption (Doc ID 1297710.1)

Symptoms

When attempting to login to the policy services UI, even though the customer was using the correct credentials the login failed with the message "Login failed -- unknown userID, password, or customer"

Changes

Re-install attempts to fix the problem. Possible wrong root customer selection on some of these re-installs
