FEM_ADI_MISSING_DIMENSION Loading Hierarchy WebADI Spreadsheet (Doc ID 1311077.1)

In Oracle Enterprise Performance Foundation Administrator (FEM) R12, you cannot open a WebADI spreadsheet to upload a Hierarchy.  When you go to Configuration Rules > Dimension > Hierarchy, select a dimension and click on the "Create from Spreadsheet" button, the spreadsheet opens but it is incomplete and the following message is on the screen:

The following error has occurred
Exception Name: oracle.apps.bne.exception.BneFataException -
oracle.apps.bne.exception.BneMissingParameterException: FEM_ADI_MISSING_DIMENSION
Log File Bookmark: 524520

In the spreadsheet, the Dimension row in the Header is blank and there is a "Level Use Code" column in the body when it should be in the Header.

Steps to Reproduce:
  1. Go to Enterprise Performance Foundation Administrator
  2. Go to Configuration Rules > Dimension > Hierarchy
  3. Select a Dimension
  4. Click on the Create from Spreadsheet button
The spreadsheet opens and the error appears.
